Records Release Authorization Form

This form can be used by students or student organizations to:
- request a scanned electronic copy of their conduct file, and/or
- waive their right of confidentiality, granting the Center permission to discuss their conduct record with an advisor.
The Family Educational Rights and Privacy Act (FERPA) protects the confidentiality and privacy of student records, which typically cannot be released to third parties (including parents) without written consent from the student.
Students may complete and submit a signed and dated Records Release Authorization Form to have an advisor (who is not involved in the complaint) of their own choosing and at their own expense, with them during the conduct process. The Center strongly encourages students to share their side of the story; therefore, it is our expectation that the student responding to charges assume the primary responsibility for presenting her/his information, with the advisor serving in a supportive and advisory capacity.
Please note that the Center will no longer be providing hard copy of requested conduct records. The Center will send scanned electronic copies of conduct records to the student's authorized e-mail address in the space provided.
